The 1.50 addon folder has to be in (psp drive letter):\PSP\GAME4xx or 3xx folder(edit: I originally had just the GAME folder). You also have to have the 1.50 official firmware update renamed 150.PBP and copied into the (psp drive letter):\ (the root directory).

What firmware are you on??

If you are on 4.01m33-2 then it goes into the PSP\GAME4xx folder
If you are on 3.xxM33 then it goes into the PSP\GAME3xx folder.

Yeah, save whatever you need to off of the mem stick and reformat with the PSP. Copy everything back to it that you want and try running it again. When you reformat with the PSP it will make all the folders that you need including the PSP\GAMExx folder. It should read everything after that.
